Personal care services allow clients to complete activities of daily living with assistance or total support. Our team at Journey Together promotes independence and dignity while providing this intimate service for our clients.
Personal Care Services include:
- Hygiene care (shaving, oral care, grooming)
- Bathing and dressing
- Transferring and/or ambulation assistance
- Medication reminders
- Light housekeeping

JTHC assists when clients go into rehabilitation facilities or hospitals for medical events. Our care team serves where needed to provide peace of mind and a familiar face in uncertain times. Companion Care
Companion Care is a service provided to individuals who require assistance to continue living independently. Our dedicated team journeys together with your loved one and assists in household management, companionship, in-home recovery, transportation, and general health and well-being.
Companion Care Services include:
- Light housekeeping: making beds, laundry, sanitizing bathrooms
- Meal preparation and planning
- Companionship: outings, memory stimulating activities/games, support during grief
- Transportation/errands: prescription pick up, dry cleaning, groceries, scheduled appointments
- Reminders/organization: birthdays, appointments, medications
- Safety and advocacy

Geriatric Care Management
With over 55 years of combined experience, our Geriatric Care Management team understands your needs! Our experience ranges from home care, geriatric care coordination, community health, nursing, nursing education, mental health, and substance abuse. We partner with your loved one to develop a personalized plan of care to meet your needs, while preserving independence and dignity. We assist where necessary with care coordination and promote quality of life.
Our team provides a wide range of care management based on our experiences with helping families to navigate their loved ones aging in place, to include:
- In-home assessments to evaluate need
- Evaluation of housing alternatives or long-term care placement
- Insurance counseling: Long-Term Care policies
- Referrals to insurance experts for Medicare and Medicaid policies
- Referrals to attorneys, physicians, financial advisors
- Referrals to community resources or products for aging in place
- Coordination of physical and mental evaluations with the appropriate medical professional
- Coordination of transportation services
- Arrangements and monitoring of home care services and/or adult day care services
- Coordination of care management for our home care services offered through JTHC
- Assistance with respite care needs
- Long distance caregiving support
- Crisis intervention
- Advocating for resident’s rights and good care practices at long-term care facilities

Frequently Asked Questions
Is your agency licensed and insured?
Yes, we are licensed by the Virginia Department of Health and we meet all requirements to provide care in the Commonwealth of Virginia. Our agency maintains all necessary insurance requirements set forth by the Virginia Department of Health. We also maintain worker’s compensation insurance for our Care Partners.
Am I contracted into services once I sign up?
No, there is no contract for services required to sign up for services.
How do I pay for services?
We work with Long Term Care insurance companies for clients with policies that cover home care as well as private pay.
Do you require a minimum number of service hours per day or week?
No, we do not have a minimum and serve 1-24 hours with flexible schedules that meet your needs.
How does JTHC screen their Care Partners?
Our Care Partners are screened and meet our guidelines and standards for employment. We perform a thorough investigation that includes personal and employment reference checks, criminal background checks, DMV record checks, drug screen, and Tuberculosis screening. Our Care Team is closely supervised and trained prior to and during service to our clients. Care Partners must be in compliance with our screening process, orientation, and any necessary training required before they serve a client. JTHC cultivates a Care Team that is healthy and eager to serve you!
